This program provides several health care options for you to obtain personal assistance. You can get help from this program if you are 60 years of age or older or if you are 18 years of age or older and have a disability. This program may help you if you are having a hard time caring for yourself.

This program provides several options for you to get help if you need personal care. In some cases, services may help you remain living at home with some support. You can also get an assessment if you are going to move into assisted living. Other services may include:
- Home-based services, such as companion care or chore and homemaking services
- Adult foster care
- Adult day care
- Nutritional counseling and meal preparation
- Screenings for those entering nursing facilities
- Social activities for you to participate in
